Proof of delivery

Your reimbursement depends on the delivery record, so we built the job around it.

A crew can't close a stop until the record is complete. No signature, no photos, or a missing item code, and the app won't let them mark the delivery done.

What every record includes

  • Item description with the HCPCS code, or the brand and model number
  • Quantity delivered
  • Date and time of delivery, which is your date of service
  • The patient's signature, or an authorized representative's signature with their name and relationship to the patient. No signature stamps.
  • Photos of the equipment placed and set up
  • GPS location and time at signature
  • Names of the crew members on the stop

How the crew captures it

Every field above is required in the crew app. If the patient can't sign, the app asks for the representative's name and relationship before it accepts a signature. If the phone has no signal inside the home, the record saves on the phone and uploads as soon as it reconnects.

After the delivery

You get the record the same day as a PDF tied to your order number. We keep every record for at least seven years and can pull any of them when a Medicare contractor asks.
